The Application

Before submitting an application we ask that you go back and read over the site for more information. It is strongly recommended to also read over the latest logs as well as read over character profiles that are in relation to your characer. Once you have done this save the example form to your PC or Mac and fill it out. When you are finished with your application please send it to queenserenitytsukino(at)yahoo(dot)com with the subject title 'Forsaken RPG application' and following that your characters name or it will be deleted. All applications will then be reviewed by the members of the game.

The Player

The Character

Name: What is your characters name?
Nickname: What is your characters nickname?
Gender: Is your character male or female?
Alliance: Is your character good, evil, or neutral?
Character Image URL: Do you have an image available of your character? Please take note that we refuse to display any pictures that were done by other artists unless you received expressed permission (and send it to us) from that artist to use it here.
Age: How old is your character? (NOTE: Inner Senshi are 20-21, Outer Senshi are 22+ excluding Hotaru)
Birth Date: When was your character born?
Astrological Sign: What's your characters sun sign? (ex: Aries, Aquarius)
Hobbies: What does your character like to do on their spare time?
Favorite Food: Self-explanatory.
Least Favorite Food: Self-explanatory.
Favorite School Subject: Self-explanatory.
Least Favorite School Subject: Self-explanatory.
Strengths: What is your character good at?
Weaknesses: What is your character not good at, and what do they fear? Please do not put N/A. Everyone has a weakness!
Goals: What is your characters long term and short term goals? (please, if you aren't an original member do NOT put "to be the strongest" or whatever.)

Physical Description

How does your character look like? Are they short, tall? What's their hairstyle and color? Are they skinny, fat, chubby? Basically, explain IN DETAIL on how your character looks like. (For Otaku Senshi, Knights, and Villains.)

Personality

How does your character act daily, around others, when they are alone, etc? Basically, explain your character's overall personality.

Realm of Influence: What is your characters realm of influence? (ex: fire, water, moon, purity, etc.)

Transformation Phrase & Sequence

(Only required for Otaku Senshi and possible Knights. Please be descriptive of the transformation as well.)

Attacks

What are your characters attacks? You must have at least 1-2 minimum. Must include a description on how the attacks work along with the attack name.

Minor Abilities

What's your characters minor abilities? Can they teleport, or what other special skill does your character have?

History

What's your characters history? Please explain IN DETAIL (3 paragraphs minimum). Must explain the past, present and future history if all three are applicable. You should also add in some information on what has changed with your character in this RPG.
